Unable to submit my reference form
This occurs when all one or more required fields are incomplete.
Please check off all required boxes (ex. Terms of Service), and sign the signature pad (if applicable) before submitting.
A “successful completion” message will pop up when the form is properly filled out.
Another possibility is network error.
